LPN - Memory Care Unit - All Shifts

Department: Nursing
Location: Goshen, IN

Provide consistent high-quality care by identifying resident needs and progress, medical assessments and promotion of team approach to resident care on Memory Care Unit. Provide support to Resident Care Specialists with resident care and charting. Believe in the values of customer-centered service, stewardship, ethical conduct and reflection, open communication, and fair treatment of all residents, family and team members. Also responsible to the residents and families served.

Essential Functions:

  • Pass medication and provide treatment for residents, including, but not limited to, tube feedings, IV medications, colostomy and ileostomy care, G-tube care, PICC line/IV care, oxygen care, monitoring and set-up and glucometer readings.
  • Check physician orders with MARs for accuracy (ie. new admissions, new orders, etc.).
  • Ensure MDS summaries are completed by their due date.
  • Be aware of the safety of residents who may roam or be unsteady and have alarms or wander guards. Answer all alarms on the community.
  • Answer call lights for all residents on the community.
  • Provide support to Resident Care Specialist/CNAs with resident care.
  • Assure all paperwork is completed in a timely manner. Receive and give reports as appropriate for resident care.
  • Attentive to Safety, Lifting and Resident Transfer Guidelines

Position Requirements:

  • Licensed as Licensed Practical Nurse by the State of Indiana.
  • Memory Care experience

Physical Requirements:

  • Frequent fingering, walking, sitting, talking, hearing and repetitive motions required.
  • Occasional reaching, kneeling, standing, pulling, lifting, grasping, feeling required.
  • Requires the ability to lift up to 80 pounds occasionally and up to 60 pounds frequently.
  • Requires the ability to see things clearly at 20 inches or less, as is common in visual inspection or computer work.
  • This position has occupational exposure to blood borne pathogens and other potentially infectious materials. Adherence to universal precautions is required.
  • Requires the ability to lift residents to a chair with the assistant of another person and a gait belt.

Schedule:

  • Every other weekend/every other holiday
    • Weekend shift pay: Additional $1.00/hr
  • Evening & Night Shift Pay: Additional $1.50/hr
